Copay-based plans: $20–$50 per visit. Coinsurance plans: typically 10–30% after deductible. High-deductible HSA plans: full clinic rate until the deductible is met, then coinsurance. We run a benefits check at intake so you know what to expect before your first visit.
Aetna typically does not require prior authorization for the initial round of outpatient PT visits. Some plans require it after a threshold (usually 12+ visits) — we handle the submission.
