Post-Surgical Rehab

Hip Replacement Rehab in Washington DC and Bethesda

Outpatient physical therapy after total hip replacement (THA). Most hip-replacement patients recover faster than knees — full weight-bearing from day one and a quicker return to walking. We pick up after home health (usually weeks 2–3) and carry you through gait normalization, strength rebuilding, and return to higher-level activity.

Phased rehab plan

Weeks 2–6

Phase 1 — Early outpatient

Normalized gait without cane, hip range of motion within surgeon precautions, climbing stairs reciprocally

Weeks 6–12

Phase 2 — Strength and endurance

Single-leg stance, full glute and hip strength, return to driving and full work

Months 3–6

Phase 3 — Higher-level activity

Walking long distances, cycling, doubles tennis, hiking, golf

Hip precautions

Anterior-approach THAs typically have no precautions. Posterior-approach THAs usually restrict deep hip flexion, adduction across midline, and internal rotation for 6–12 weeks. We follow your surgeon's specific instructions and teach you how to bathe, dress, and get out of cars without violating them.

What evaluation looks like

Sixty-minute evaluation with a Doctor of Physical Therapy. We review your op report, screen gait, measure available range of motion, and check glute and hip strength. First treatment same visit.

Treatment approach

Early phase: gait retraining, glute activation, hip range-of-motion work within precautions, balance. Mid phase: progressive single-leg loading, stair training, return to driving prep. Late phase: higher-level strength and endurance, return to recreational activity.

Frequently asked questions

When can I drive after hip replacement?

Most patients with a right THA are cleared between weeks 3 and 6. Left THA patients with automatic transmissions often return sooner. We test before clearing.

How long is hip replacement rehab?

Most patients finish formal PT around 8–12 weeks. Strength and full activity tolerance continue improving for 6–12 months.

Can I still play golf or tennis after hip replacement?

Most patients return to golf, doubles tennis, hiking, cycling, and swimming. High-impact running and singles tennis are more variable — we'll talk through realistic expectations during your evaluation.

Do you accept Medicare for hip replacement rehab?

Yes. In-network with Original Medicare and most Medicare Advantage plans at all three locations.
