Knee Replacement Rehab in Washington DC and Bethesda

Outpatient physical therapy after a total knee replacement (TKA). We pick up where home health leaves off — usually around weeks 2–3 — and carry you through return to walking, stairs, driving, and eventually return to higher-level activity. Same-week starts at Capitol Hill, Bethesda, and in-home throughout the DMV.

Phased rehab plan

Weeks 2–6

Phase 1 — Early outpatient

Knee extension to neutral, knee flexion to 110°+, normalized gait without an assistive device, swelling control

Weeks 6–12

Phase 2 — Strength and endurance

Quad strength, single-leg stance, full stair reciprocation, return to driving and full work duties

Months 3–6

Phase 3 — Higher-level function

Return to walking long distances, cycling, golf, doubles tennis, recreational hiking

Goals we target

Range of motion is the time-sensitive priority — most surgeons want full extension to 0° and flexion to at least 110–120° by the 12-week mark. Strength catches up after motion. We measure both at every visit and progress your home program accordingly.

What evaluation looks like

Sixty-minute initial evaluation with a Doctor of Physical Therapy. We review your op report, measure range of motion and strength, look at gait and stair mechanics, and check the surgical site. First treatment same visit. Plan of care is usually 2–3 visits per week for 6–8 weeks, then tapering.

Treatment approach

Manual therapy to maintain motion gains and address soft tissue tightness, progressive loading (open- and closed-chain quadriceps work, hamstrings, glutes, calves), gait retraining, stair training, and balance work. Modalities (ice, e-stim) when appropriate for pain and swelling — not as a main course of treatment.

Insurance and costs

Medicare covers TKA rehab under Part B (80% after deductible). Most commercial plans cover it — we verify benefits at intake. Self-pay $150 per session.

When should I start outpatient PT after my TKA?

Most patients start outpatient PT around weeks 2–4. If you're using home health right now, we typically pick up when you transition off it. Call us and we'll coordinate.

How long does TKA rehab take?

Most patients finish formal PT around weeks 10–14. Full strength and endurance recovery continues for 6–12 months — we'll send you off with a maintenance program.

Do you accept Medicare for TKA rehab?

Yes. We're in-network with Original Medicare and most Medicare Advantage plans at all three locations.

Can I get PT at home after my knee replacement?

Yes — our In-Home program serves DC, Maryland, and Virginia. Same Medicare and commercial coverage applies.

When can I drive again?

Most patients with a right TKA are cleared to drive between weeks 4 and 6, depending on range of motion, strength, and reaction time. We'll test you before clearing.
